1.INTRODUCTION

• ◆Doctors and surgeons

• ◆Attorneys and legal professionals
• ◆Transportation providers

By creating a provider account, you agree to these Terms.

2.ROLE OF MMONAJAH

• ◆A medical provider or healthcare facility
• ◆A law firm
• ◆An employer
• ◆A supervisor of your services
• ◆A party to the service agreement between you and the Member

You operate as an independent contractor, not an employee.

3.PROVIDER ELIGIBILITY

• ◆Be at least 18 years old
• ◆Provide accurate personal and business information
• ◆Hold all required licenses or certifications for your services
• ◆Maintain liability insurance (if applicable)
• ◆Pass identity verification, and provide government-issued identification if you are applying for a specialist or board role
• ◆Agree to these Terms

You are responsible for keeping your credentials up to date.

4.ASSOCIATION MEMBERSHIP & DUES — YOUR ONLY FEE

• ◆Mobile Providers
• ◆Standard Wellness
• ◆Medical, Dental & Legal
• ◆Facilities & Surgical Centers

Association dues are your only annual fee. MMONAJAH charges no platform membership fee, no platform subscription fee, no listing fee and no setup fee to providers, doctors, therapists, attorneys or facilities. Dues are non-refundable and billed annually (or as a one-time Founders 15-Year payment where offered). Failure to pay within 30 days of renewal may result in account suspension.

5.WHOLESALE PRICING — NO COMMISSION

• ◆Not to circumvent the platform
• ◆Not to accept off-platform payments from Members
• ◆Not to solicit Members outside the platform

Violations may result in account termination.

6.PROVIDER RESPONSIBILITIES

• ◆Provide safe, professional, high-quality services within your license and scope of practice
• ◆Arrive on time for all bookings
• ◆Maintain clean, sanitized equipment
• ◆Communicate clearly with Members
• ◆Follow all applicable laws, regulations and professional obligations
• ◆Respect Member privacy and confidentiality
• ◆Report safety concerns immediately

You are solely responsible for your conduct and services.

7.BOOKING & SERVICE DELIVERY

• ◆You must confirm or decline promptly
• ◆You must show up at the scheduled time
• ◆You must complete the service as described
• ◆You must mark the service as completed in the app

Failure to complete bookings may affect your rating or account status.

8.REAL-TIME TRACKING

• ◆Mark "On the way"
• ◆Use real-time tracking
• ◆Mark "Arrived"
• ◆Mark "Service started"
• ◆Mark "Service completed"

This ensures transparency and safety.

9.PAYMENTS & PAYOUTS

• ◆100% of the wholesale rate you set, paid directly through Stripe when the Member's booking is paid
• ◆Tips (100% to provider)
• ◆Bonuses (if applicable)

Stripe pays you directly at the time of each booking, so there is nothing to wait for. No platform fee, subscription fee or commission is deducted from your rate. MMONAJAH is not responsible for bank delays, incorrect payout information, or failed transfers due to provider error.

10.CANCELLATIONS & REFUNDS

• ◆The Member may receive a platform credit or help rebooking
• ◆The Provider may receive a penalty
• ◆Repeated cancellations may lead to suspension

Cancellation is permitted in many cases, but no refunds are issued — see the Refund Policy and Cancellation Policy. Association dues and Founders 15-Year payments are non-refundable, and Provider compensation follows the platform's cancellation rules.

11.SAFETY & COMPLIANCE

• ◆Follow all safety protocols
• ◆Report infections, complications, or concerns
• ◆Never diagnose or prescribe unless licensed
• ◆Never perform services outside their scope

MMONAJAH may suspend or remove Providers who violate safety standards.

12.INDEPENDENT CONTRACTOR STATUS & TAXES

• ◆You are not an employee of MMONAJAH
• ◆You set your own schedule
• ◆You set your own prices (within platform guidelines)
• ◆You provide your own equipment
• ◆You are responsible for your own taxes and licenses
• ◆You will provide a completed W-9 before any payment is issued to you
• ◆MMONAJAH reports payments as required by law

Nothing in these Terms creates an employment relationship.

13.PROHIBITED CONDUCT

• ◆Harass or harm Members
• ◆Discriminate against Members
• ◆Provide unsafe or illegal services
• ◆Circumvent the platform
• ◆Misrepresent credentials
• ◆Share Member information
• ◆Engage in fraud

Violations may result in immediate termination.

14.ACCOUNT TERMINATION

• ◆Safety violations
• ◆Fraud
• ◆Poor performance
• ◆Repeated cancellations
• ◆Off-platform solicitation
• ◆Non-payment of Association dues

You may cancel your account at any time. Association dues already paid are non-refundable.

15.LIMITATION OF LIABILITY

• ◆Your services
• ◆Your conduct
• ◆Injuries or damages caused by you
• ◆Loss of income
• ◆Member disputes

To the fullest extent permitted by law, MMONAJAH's total liability to you is limited to the Association dues you paid in the twelve months before the claim. You agree to indemnify MMONAJAH against claims arising from your services.

16.DISPUTE RESOLUTION

These Terms are governed by the laws of the State of Florida. Disputes between you and MMONAJAH will be resolved by binding individual arbitration after a 30-day good-faith negotiation period, and each party waives the right to a jury trial and to participate in a class or representative action. Either party may seek injunctive relief in court to protect confidential information.

17.MODIFICATIONS TO TERMS

• ◆Continued use of the platform constitutes acceptance
